All new patients wishing to register must provide 2 ITEMS SHOWING PROOF OF ADDRESS dated within the last 3 MONTHS to verify your where you live. Utility bills are ideal however please do not hesitate to contact the surgery for more information on what we can accept.
For children aged 5 or under, parents must provide their NHS number and their red books so the surgery can take a copy of their vaccination records before being registered.
All new patients aged 16 and over must have a new patient health check as this is part of the registration process. If you change your address or telephone number please inform the surgery as soon as possible.
