Order A Repeat Prescriptions
Repeat prescriptions are issued using a computerised system. A repeat medication request form is attached to each prescription with details of your medication and the approximate date you should see your doctor for a review.
Until the review date tick the items required when you need a further supply.
Either insert this in the prescription box at the reception or post it to the surgery, in which case a stamped addressed envelope should be enclosed if you wish it to be returned by post. Please allow two working days. Please also allow time for postal delays.
When your review is due please make an appointment to see the doctor.
We regret that prescription requests cannot be accepted by telephone as this can lead to dangerous mistakes. If you have lost, or do not have a repeat prescription form, then write down your doctor's name, your own name and address and the name and strength of the medication required and how often you take the item you wish to be repeated. Then post this note in the reception box.
