How To Register
How To Register With The Practice
All new patients wishing to register must check whether they fall within the catchment area of the practice. No Proof of address or ID is required to register. If you do have your previous NHS card please bring this with you as it enables us to locate and retrieve you medical records as quickly as possible.
For children aged 5 or under, parents must provide their NHS number and their red books so the surgery can take a copy of their vaccination records before being registered.
All new patients aged 16 and over must have a new patient health check as this is part of the registration process. If you change your address or telephone number please inform the surgery as soon as possible.
